If you are shopping for delicious wines with beautiful, high-quality, custom designed labels, you have found the right place! Orders may be placed online or print this fax back page, indicate your selections and contact information and fax your order to (909) 899-9196 or email to jaredpfilippi@yahoo.com.

1. You may provide your artwork or pick from our selections
2. Labels are available in sizes:
   • 4" width x 3" height
   • 3" width x 4" height
   • 3" width x 2.5" height
   • 2.5" width x 3" height
3. Dry Champagne/Almond splits 3" width x 2.5" height required.
4. 750 ml Dry Champagne/Almond 4" width x 3" height required.
5. Choose a label style, provide your own artwork, or tell us what you'd like.
6. Provide contact number(s) and best time to call, we will call to arrange transfer of photos.
7. Custom label wine available by the case only - 12 bottles (except splits, cs of 24)

The custom labels will not be designed and printed until the wine and labels have been paid in full.
Initial layout & up to 2 corrections are included at no additional charge; thereafter $30.00.

Pick up at Rancho Cucamonga tasting room or shipping is available at standard shipping rates. Indicate your desired greeting, message, or names below. If you have specific "font or colors" desired, please indicate. We recommend you visit the tasting room to place your order.
